此經記述文殊菩薩代四十九位新發意菩薩，請示佛說長壽法。佛遂說過去無量劫有一顛倒女殺子傷胎，犯世間五種難懺罪之一：一者殺父，二者殺母，三者殺胎，四者出佛身血，五者破和合僧。顛倒懊悔不已，懇請佛說滅罪法。佛於是說諸佛長壽命經，並告知顛倒女及一切眾生，若有犯五逆罪眾生，能受持書寫讀誦此經，至誠懺悔，當滅眾罪。經中佛又說護諸童子陀羅尼咒，示眾生護養童子之法。
末世眾生視墮胎為常事，不知因果，不畏惡道。被墮之胎兒，則含冤莫申，怨氣不散，為導致世界紛亂原因之一。此經特別標出墮胎之嚴重性，足以警惕愚迷莫再行顛倒之事。
This sutra describes how Manjushri Bodhisattva, on behalf of forty-nine Bodhisattvas of initial resolve, requests the Buddha to speak on the Dharma of obtaining longevity. The Buddha tells of how limitless eons in the past, there was an upside-down woman who killed her children and aborted her babies, thus committing one of the five grave offenses which are hard to repent: 1) killing one's father, 2) killing one's mother, 3) killing a fetus, 4) shedding the Buddha's blood, and 5) disrupting the harmony of the Sangha. She was filled with regret and sincerely requested the Buddha to speak the Dharma to eradicate offenses. Therefore the Buddha spoke the Sutra for Obtaining Longevity and Eradicating Offenses, and told the upside-down woman and all living beings: should one have committed the five rebellious offenses, if he can uphold, write, read, and recite this sutra and earnestly repent, then all offenses can be eradicated. In the Sutra, the Buddha also spoke the Dharani for Protecting Youths, showing living beings the way to protect and care for youths.
In the Dharma-Ending Age, living beings regard abortion as something normal; they do not know about the law of cause and effect, and are not afraid of the evil paths. The aborted babies harbor unredressed grievances, and their enmity does not dissolve over time; this is one of the reasons for the chaos in the world. This Sutra especially emphasizes the seriousness of abortion, warning deluded and confused people not to commit this upside-down act.
